You might be interested in using Dragon for voice capture to text. I have more wrist problems than anything and I solved mine by using a silicone wrist pad. My wrists rest on it and that is the only part of my body that touches anything except the keys.
I wonder if your height and arm length play a role in the elbow thing.
I tend to use a keyboard with a long cord or wireless, so I can place it in my lap. They're both fairly cheap. Voice recognition software can be helpful, but it takes a serious time commitment before the system “learns” your speech patterns and inflections.
I developed tendonitis in my right elbow from the position I used the mouse. Learned to mouse left-handed but I wasn't writing a book. :-0
